Pro-D Days (Limited Spaces)

Complete the secure online form below. The website will place your information into the official two-page Pro-D registration PDF and email it directly to Strawberry Vale OSC. Your request is not confirmed until the team follows up.

2. Requested care dates

Select at least one eligible full-day care date (required). Hours are 7:30 AM to 5:30 PM. Current space is shown for each date below.

Fee: $65.00 per child, per selected date

Pro-D Day fees are due only when an SVOSC admin contacts you by email from oscstrawberryvale@gmail.com requesting e-transfer payment. Available spaces remain pending until OSC staff record payment. Waitlisted dates are not charged.

Open the current SVOSC policy handbook at page 7I opened the policy handbook and understand Pro-D Day fees are due when requested by SVOSC and are non-refundable once paid.

I understand this request holds space with payment pending and is not confirmed until OSC confirms that payment has been received.If a selected date is full, I agree that the child may join that date's waitlist without being charged. OSC staff will contact me if a space becomes available.
