Pro-D Days (Limited Spaces)
Download and complete the Pro-D Care Form, then attach it to an email to request registration and check availability. Spaces are limited and your request is not confirmed until our team follows up.
How to request Pro-D Day care
- Open and complete the Pro-D Care Form.
- Click the email button to open your email app.
- Attach the completed form and send the request to oscstrawberryvale@gmail.com.
The email will be addressed to oscstrawberryvale@gmail.com with the subject “Pro-D Days Registration Request” and attachment instructions already included.
